create pdf reports based on excel column value

I want to create separate pdf files based on excel column value...

For example, excel sheet as follows:

Date Name Gross From Email Address
1/1/2016 ABC $100 abc@gmail.com
1/1/2016 def $50 def@gmail.com
1/10/2016 lmn $100 lmn@yahoo.com

Expected results

Pdf file # 1

Name: ABC
Amount : $100/-
email Address: abc@gmail.com

Dear Abc,

PDF file # 2

Name: DEF
Amount : $50/-
email Address: def@gmail.com

Dear def,

Thank you for your donation......

PDF file # 3

Name: LMN
Amount : $100
